There should be an accompanying kernel message. Also the core dump itself
should tell you something. That's what they're for.

My experience of virtual private servers is one of very very variable I/O
performance. Another thing that bites quite often is Linux overcommitting
on virtual memory and killing the wrong process when it goes wrong. But
again logs should say so.
