Everything you want, including this point, can be achieved using Sympa
<URL:http://www.sympa.org/>. The policy I choose on my own big mailing-lists
is:
  - subscribers can post freely as long as the list is explicitely in To or Cc
  - other mails must be confirmed by answering a challenge
  - confirmed mail goes through moderators and one of them has to accept it

I get zero spam mailing-lists this way. Also, through the confirmation
process, moderators do not see spam either.
