Fix error handling of httpc_manager and its starter process

Hi,

Here is a patch that fixes error handling of httpc_manager when there's
a problem with the starter process or httpc_handler.

The symptoms were either no answer sent to the caller or multiple
answers sent for the same request.

The branch is named "inets_httpc_manager_error_handling":
