Patch-submitting instructions updated

We have updated the wiki page on how to submit patches:

http://wiki.github.com/erlang/otp/submitting-patches

and it should now be fairly complete. There is also a new
page that describes the branches in the repository:

http://wiki.github.com/erlang/otp/branches

There is also a new page started by thijsterlouw that describes
the layout of the source tree:

http://wiki.github.com/erlang/otp/routemap-source-tree


If you have sent patches to us in the past that have not been included
or even acknowledged, we kindly ask you to submit them again
following the new instructions. I think we should be able to take care
them in a better way now.
