Prebuilt documentation

A prebuilt version of the documentation is checked in under the
directory
prebuilt_doc. The top index file is prebuilt_doc/doc/index.html.
This is done due to that the Corba documentation is still in the old
format
and therefor needs to be built with OTP 26.

complete license headers have been added to some files
that was missing them.

Buffer overflow bug in IC C library

A buffer overflow problem was fixed in oe_prepare_request_decoding()
in IC C library. The function is designed just to handle calls from
a gen_server and therefor only handles "$gen_call" and "$gen_cast".
The problem was that it didn't take care if an atom longer than it's
buffer came instead of these two atoms.
