There's a fair bit of detail about this in Kirk McKusick's chapter in
the _Open Sources_ book.
If you haven't heard his History of BSD presentation (at USENIX or
other venues), it's worth hunting down. Hrm. He appears to have a
video of that lecture available at
http://www.mckusick.com/history/index.html. What a deal.
