[erlang-patches] Small dyntrace.erl cleanup patch

Hi, sorry I didn't submit this sooner.  It's a couple small bits of
cleanup for dyntrace.erl, based on the "maint" branch.

    dyntrace.erl comment cleanup and unused function removal
    
    Change the module-level docs to give complete step-by-step
    instructions for using the `dyntrace:p()` trace function.  Also,
    remove the unused `scaff/0` function.
