I just realized that I completely failed to mention that I've been
doing my testing on R15B01. The same code exists on the head of the
OTP github repo, though, so I assume the behavior still exists in the
latest release.

I have also just added notes to the Riak Pipe issue for easily
reproducing the original issue in the wild.
