[erlang-bugs] Documentation: additions to Index

That's great thanks.
I've been going through some of the docs and recording things that can
be repaired or updated. I will send it through in the weeks to come.

For now, more index additions:
Add "SMP" to the index;
There are probably more keywords missing from the "erl" module in the index.

Documentation is very important for people picking up the language for
the first time.  Python's nice documentation is probably one of the
reasons why the uptake has been so great.

I agree that the Modules page also needs to have the "A B C... Z"
anchors at the top.
